Stabilization

Stabilization

Stabilization occurs once our Care Managers have implemented the plan that addresses the immediate needs of our clients. Care Mangers then shift their focus to the long-term goals that are important to our clients. Our team works with clients to help them design future plans and help them to reach their goals.

Adult Guardianship

Adult Guardianship is a decision of last resort. It is needed when a person is unable to make and communicate responsible decisions regarding his personal care due to a mental, physical or developmental disability. The biggest benefit of adult guardianship is that it offers peace of mind for all parties. The TAG Team will step in to make decisions about medical care, housing, social services, and other needs that focus to the highest quality of life for our clients.

Elder Mediation

Communication snags in the care of a loved one? Conflict tearing your family apart? Allow TAG to assist you and your family toward a more peaceful journey through elder mediation.

Mediation focuses on topics of money and financial matters, strengths and vulnerabilities, caregiving and support, end-of-life healthcare decisions, and everything in between. Our role as mediator is not to make the decision for the family but listen and help the concerned parties openly discuss issues that help lead to resolution.
