Care managers conduct in-depth assessments, develop care plans and recommendations, coordinate services, act as liaisons to health care providers and insurers, and continuously monitor services to ensure that the individual’s goals are met.
Assessment is conducted in-person and usually one-on-one. We value the input of caregivers, family, and other loved ones to help us round out the client’s viewpoint and fill in missing pieces of our client’s story. We use best practice tools to evaluate our clients and allow the data to help us make informed recommendations and develop care plans.
Care plans are our roadmap. We design our care plans using our clients’ goals, the results of our assessment, and our professional experience to build a plan, which addresses immediate, short-term, and long-term solutions. These goals are reviewed with our clients and guide our work together.
Care monitoring and reports keep us connected to our clients and holds us and our clients accountable for the plans we designed together. We meet with our clients to provide support along their care journey and we update the care plan, as needed. We issue regular progress reports to summarize our work together and proactively schedule the next monitoring date with our client.
